The Ultimate Guide to Basic Life Support: What to Do When Someone Collapses

It is the scenario we all dread. You are at the shops, at work, or walking down the street, and someone suddenly collapses.

They aren’t moving. They aren’t responding.

In that moment, you are the most important person in the world. You are more important than the ambulance crew, and more important than the doctors at the hospital. Why? Because you are there.

For every minute that passes without CPR, a person’s chance of survival drops by 10%. By the time the ambulance arrives, it might be too late. You are the bridge that keeps them alive.

Many people freeze because they are afraid of doing it wrong. As a doctor, let me tell you the golden rule of resuscitation: Doing something is always better than doing nothing.

The Action Plan: DR ABC

Follow these steps in order.

D

Danger

Before you rush in, stop. Look around. Is there broken glass? Electrical cables? Fast-moving traffic? Ensure the area is safe.

R

Response

Check if they are conscious. Kneel by their side, gently shake their shoulders, and shout loudly: “Hello! Can you hear me?” If no answer, they are Unresponsive.

A

Airway

Place one hand on their forehead and two fingers under their chin. Gently tilt the head back to open the airway.

Breathing

Look, listen, and feel for normal breathing for no more than 10 seconds.

⚠️ Warning: Infrequent, noisy gasps are NOT normal breathing (“agonal breathing”). If unsure, assume they are NOT breathing.

Call & CPR

If unresponsive and not breathing: Call 999 (or 112). Ask for an ambulance and an AED. Start CPR immediately.

  • Position: Kneel next to the chest.
  • Hands: Heel of one hand in the center of the chest. Interlock fingers.
  • Push: Push down hard (5-6cm deep) and let the chest recoil fully.
  • Speed: Push fast (100-120 beats per minute).

Hum “Stayin’ Alive” or “Baby Shark” to keep the perfect beat.

⚡ The Defibrillator (AED)

Don’t be scared. It talks to you.

You cannot kill someone with an AED; you can only save them. It will not shock unless it detects a shockable rhythm.

  1. Turn it on.
  2. Stick the pads on the bare chest (follow pictures).
  3. Listen to the voice commands.

What If They ARE Breathing?

The Recovery Position

If they are unconscious but breathing normally, do NOT do CPR. Place them on their side to keep their airway safe.
